Managing Costs by Managing Care

North Park Nursing and Rehabilitation Center is credentialled by Extendicare Health Facilities, Inc. to provide consistent and predictable clinical outcomes. By credentialling its centers, Extendicare clinical teams are able to deliver a wide range of services within the credentialled level. This means patients with complex medical conditions are placed appropriately.

North Park manages costs by managing care through its daily evaluation of each patient's acuity - using body system assessments, comorbidities and medical/rehabilitative conditions to determine treatment planning. Our patient classification system closely approximates systems used in acute care hospitals, and enables us to prospectively and concurrently monitor the cost of caring for patients as their conditions change during the course of their inpatient stay.

Daily Services

Our daily room rate includes the following services:

24-hour nursing care and supervision
Basic respiratory care including oxygen administration
Restorative nursing services
Nutritious meals, snacks, and special diets
Housekeeping and complete laundry services
Therapeutic adult recreational activities including religious services
Social services available to assist families and residents

Additional Services Available

Many other services are available at reasonable charges, and may be covered by your individual insurance. The following services are not included in our daily rate:

Physician services - the resident's personal physician is encouraged to continue to follow each residents plan of care
Dental, optometry, and podiatry services
Physical, occupational, and speech therapies
Medical supplies
X-ray and laboratory tests
Medical equipment such as wheelchairs, walkers, and adaptive devices
Personal telephone service
Cable for individual rooms

We admit patients 24 hours a day, 7 days a week, to best accommodate the ever-changing needs of our referral sources and their patients. Patients are also admitted from home, physician offices, hospital emergency rooms or urgent care centers. We understand that each family may have special demands that require our admitting team to be flexible with availability.
