| Skilled medical services are suitable for
patients whose activities of daily living cannot be adequately provided for in a home
setting. These patients are medically stable, but may have a limited capacity for
therapies. Patients may also require skilled medical services as a result of a
debilitating state either due to surgical intervention, a prolonged acute illness or a

Under the direction of a personal physician or our medical director, our interdisciplinary team of healthcare professionals develops and delivers an individualized plan of care for each resident. Alzheimers Pavillion at North Park The Pavillion at North Park provides the least restrictive environment for Alzheimers' residents. We are proud to offer peace of mind to the families of our residents by enabling the residents to enjoy the maximum amount of movement and expression in a home-like environment. The quieter environment enables residents to maximize their independence while minimizing the frustration often associated with Alzheimers disease. Our special Alzheimers program includes:
Care for the resident begins with an assessment of each person's individual needs in conjunction with the attending physician. We continue by evaluating each resident frequently to ensure we are effectively meeting their emotional, psychosocial and medical needs. A program director dedicated specifically to this special care program and ongoing staff training are both essential elements in the care we provide to each resident. Our staff has the training and experience, the patience, and compassion to work effectively with those who have Alzheimers disease. Skilled Rehabilitation Services Rehabilitation patients may require a wide range of services, depending on their individual condition and diagnosis. Skilled rehabilitation services are included in the plan of care for patients who are in a stable condition and would benefit from physical, occupational and speech therapies. Our skilled rehabilitation program is customized to meet the needs of each patient as they progress through recovery. The goal of our skilled rehabilitation program is to increase the independent functioning of the patient and attain their highest possible level of recovery.
