Therapy Room

Skilled medical services are suitable for patients whose activities of daily living cannot be adequately provided for in a home setting.  These patients are medically stable, but may have a limited capacity for therapies.  Patients may also require skilled medical services as a result of a debilitating state either due to surgical intervention, a prolonged acute illness or a chronic disease.

General and Restorative Services

We are committed to accommodating our residents' healthcare needs by providing value in quality services.  Our center is staffed by dedicated, qualified, licensed healthcare professionals who offer compassionate care for our residents.  Through our friendly and personalized services, we recognize each individual's unique needs in a respectful, dignified and nurturing manner.

Long-term care is designed for those who cannot be cared for in a home setting due to illness, injury, frailty or other physical limitations.  Under the direction of a personal physician or our medical director, our interdisciplinary team of healthcare professionals develops and delivers an individualized plan of care for each resident.

We provide a full range of general and restorative services to promote the well-being and independence of our residents, including:

  • 24-hour comprehensive nursing care
  • On-call physician coverage
  • Private or semi-private rooms and furnishings
  • Medication administration
  • Social services
  • Goal-oriented restorative programs
  • Functional activities
  • Pharmaceutical consultant
  • Nurse Practitioner on staff to assist our residents

Alzheimers Pavillion at North Park

The Pavillion at North Park provides the least restrictive environment for Alzheimers' residents.  We are proud to offer peace of mind to the families of our residents by enabling the residents to enjoy the maximum amount of movement and expression in a home-like environment.  The quieter environment enables residents to maximize their independence while minimizing the frustration often associated with Alzheimers disease.   Our special Alzheimers program includes:

  • Spacious semi-private rooms
  • Cozy dining room
  • Recreation area
  • An enclosed courtyard where residents are able to enjoy the outdoors

Care for the resident begins with an assessment of each person's individual needs in conjunction with the attending physician.  We continue by evaluating each resident frequently to ensure we are effectively meeting their emotional, psychosocial and medical needs.  A program director dedicated specifically to this special care program and ongoing staff training are both essential elements in the care we provide to each resident.  Our staff has the training and experience, the patience, and compassion to work effectively with those who have Alzheimers disease.

Our philosophy focuses on assisting each resident in maintaining an optimal level of self care and independence.  Activities are individually designed to enhance each resident's quality of life.  Activities consist of a structured program based on the needs and abilities of the residents.  These are just a few of the activities that the residents enjoy:

  • Morning coffee
  • Exercises, ball tossing, and balloon volleyball
  • Music and relaxation time
  • Van outings and walks
  • Gardening, games, reminiscing groups, baking and cooking groups
  • Movies, sing-a-longs and news on TV

Skilled Medical Services

Skilled medical services are suitable for patients who may suffer from a prolonged acute or chronic illness or may have recently been injured or undergone surgery.   These patients are stable but continue to require assessment and treatment.   The patient may require nutritional support or assistance with self-care and activities of daily living. 

Respiratory Services.   Respiratory services benefit patients with respiratory impairments who may require a wide range of services, depending on their individual condition and diagnoses including:

  • Oxygen administration
  • Tracheostomy care
  • Pulmonary hygiene

Comprehensive Wound Care Services. Comprehensive wound care services are suitable for patients who have wounds incurred as a result of surgical intervention, trauma, disease processes, or unrelieved pressure.   Wound care management techniques include:

  • Local surgical and non-surgical procedures
  • Nutritional support
  • Prevention protocols
  • Systematic administration of antibiotic or other pharmaceuticals
  • Completion of a standardized treatment plan derived from researched best practice protocols which have demonstrated optimum healing

Specialty Services.   Specialty services at North Park Nursing and Rehabilitation Center include:

  • Infusion therapies that benefit patients suffering from dehydration, digestive disorders, nutritional deficiencies, infections, cardiac disorders, diabetes, cancer or renal failure including the intravenous administration of antibiotics, maintenance of intravenous lines, pain management, and parenteral nutrition
  • Cardiac recovery services including post-cardiac surgery and cardiac arrest recuperative care
  • Renal disease services that benefit patients suffering from acute or chronic kidney disorders, and may include hemodialysis
  • Post-intervention cancer services benefit patients receiving treatment for cancer who may develop symptoms which require skilled medical services including hydration, pain management, and post-treatment care of radiation and chemotherapy
  • Digestive disease services benefit patients who have disorders of the esophagus, stomach, duodenum, or small/large intestines, typically through the administration of enteral or total parenteral nutrition and antibiotic therapy

Skilled Rehabilitation Services

Rehabilitation patients may require a wide range of services, depending on their individual condition and diagnosis.  Skilled rehabilitation services are included in the plan of care for patients who are in a stable condition and would benefit from physical, occupational and speech therapies.  Our skilled rehabilitation program is customized to meet the needs of each patient as they progress through recovery.  The goal of our skilled rehabilitation program is to increase the independent functioning of the patient and attain their highest possible level of recovery.

For example, a typical skilled rehabilitation patient may be suffering from one or more of these diagnoses:

  • Stroke
  • Amputation
  • Trauma
  • Orthopedic disorder
  • Severe arthritis
  • Neuromuscular disorder

Physical Therapy.  The goals of our physical therapy program are:

  • To increase the strength and flexibility of joints and muscles through carefully determined exercises
  • To help muscles resume proper functioning following a brain or nerve injury
  • To reduce pain through heat massage, aromatherapy and whirlpool treatment

Occupational Therapy.  Our therapists teach patients the skills needed to achieve independence in the following areas:

  • Performing daily activities such as eating, dressing, and bathing
  • Developing and maintaining work and leisure activities
  • Improving joint mobility, muscle strength, and physical endurance

Speech Pathology.  Our therapists specialize in assessing speech and language disorders and provide the appropriate therapy to promote maximum speech function.  Some of the services include:

  • Improving receptive and expressive language skills
  • Developing proper control of articulatory and respiratory system for improved speech
  • Treatment of swallowing disorders

Respite Care

Our respite care program is a short-term temporary solution for caregivers who need a break or relief from providing full-time care to a family member or loved one.  Our program includes:

  • Furnished private or semi-private rooms
  • Physician-directed nursing care and supervision, including medication administration and therapies
  • Assistance with meals, grooming, dressing and ambulation
  • Three nutritious meals a day
  • Recreational and social service programs

Our interdisciplinary team effectively motivates our patients to meet the goals of their plan of care and return to home or work.
