Description North St. Paul Transitional Care Center (NSPTCC) is a
unique 32-bed transitional care facility providing short term care for
the subacute and rehabilitation needs of our patients. Transitional Short Term Subacute Care As one of the first facilities to provide transitional care outside a traditional hospital setting, NSPTCC has been providing subacute care to our patients for over 10 years. We provide patient care in an intimate friendly setting on a cost effective basis. Conveniently located and providing safe surroundings in a quiet residential neighborhood, we are only one block from a park and two blocks from a school. We design transitional subacute care for persons who have had an acute illness, injury, or exacerbation of a disease process. Treatment is provided either immediately after, or instead of, an acute hospital stay. An interdisciplinary treatment team--including physicians, nurse practitioners, dietitians, therapists and other health care professionals--assesses the needs of the patient, and an individualized care plan is implemented. Discharge planning is considered a key component to the care planning process. Nursing and Rehabilitation Services Our nursing and rehabilitation services are fully integrated. Therapists and nursing personnel work together to provide patient-centered and outcome-oriented results. Nurses and therapists are our employees and are located on site, providing physical therapy seven days a week, occupational therapy seven days a week, and speech therapy five days a week. Our skilled and caring staff strives to provide a high level of patient satisfaction at all times.
